Insurance Coverage and Financial Arrangements

At the time of your visit, please be prepared to pay any insurance co-payments you may be responsible for. Also you should bring any referrals from your Primary Care Physician if required by your insurance plan. We will glady file a claim on your behalf directly to your insurance company. We do not, however, bill third parties for automobile accidents or personal injuries.

In the event your insurance changes, we ask your cooperation in notifying our Business Office of your new coverage. Our Business Office staff will also work with you should extenuating circumstances prevent you from paying your account when due. Please call 288-2830 to speak with a patient account representative.
